Large drain cover rocking increasingly and cracks

Sent to Royal Borough of Greenwich less than a minute later. FixMyStreet ref: 5768720.

There is a drain cover that was recently worked on, it is rocking every time a car drives over it. it is becoming increasingly louder and there are cracks around it on the road. it looks like it is going to give way. Aside from that it is incredibly noisy and it is a main road. I can hear it from 8 floors up about 50 metres away
